Where to place a heating pad for back, knee and period pain

Short answer

Put heat over muscle, not over bone or a joint surface. Lower back: across the lumbar curve on either side of the spine. Knee: wrapped so the heat sits above and below the kneecap rather than on it. Period pain: below the navel. Keep fabric between the pad and your skin.

Placement matters mechanically. Where muscle sits between skin and bone, heat spreads and feels good. Where there is only skin over bone, the same pad concentrates heat in a small patch, and that is where burns start.

The rule: heat muscle, not bone

Foot and ankle Across the arch and around the ankle, over a sock. Numb feet, and wraps tight enough to press into the skin.

Lower back

Back pain has the most evidence behind it. The Cochrane review of superficial heat and cold for low back pain covered nine trials and 1,117 participants: moderate evidence that a heat wrap reduces pain and disability a little in the short term for acute and subacute back pain, and that adding exercise helps further.

  1. Sit or lie so the lower back is relaxed rather than stretched. A heated waist wrap holds the heat in one band across the lumbar curve, and a lumbar cushion set keeps the spine restful.
  2. Cover the muscle on both sides of the spine, not the spine itself, with a towel or clothing between pad and skin.
  3. Set 15 to 20 minutes on the lowest warm setting, then move gently while the muscle is warm. The exercise is what adds to the heat.

Knee

A knee is two problems in one place. The muscle above it responds well to heat when tight; the joint surface beneath the kneecap has almost no tissue over it, so heat there is concentrated. Wrap rather than press: a heated knee support wrap covers the front and sides evenly. For a knee that is stiff rather than swollen, 15 to 20 minutes before you need to move is reasonable.

Swollen knee, cold not hotIf the knee is swollen, warm to touch or bruised, heat is the wrong tool for that day. Harvard Health advises against heat on an area that is swollen, red or hot, or an injury just sustained, because it can increase inflammation. Use ice first and read our heat or ice guide.

Period pain

Mayo Clinic advises a heating pad, hot water bottle or heat patch on the lower abdomen to ease menstrual cramps. The trial evidence is more specific: a heated abdominal patch worn for about 12 hours a day relieved period pain about as well as ibuprofen 400 mg three times a day, and a separate trial found a wearable heat wrap beat acetaminophen over 8 hours.

A plug-in pad on the belly follows different rules: 15 to 30 minutes, skin checked, off when the timer ends. Neck, shoulders, feet and hands

Spots to leave alone

Do not put heat on theseBroken skin, a rash, or a fresh bruise of unknown cause. A numb or reduced-sensation area, including feet affected by diabetic neuropathy: a published case series describes severe burns in people who used heat for warmth and could not feel the damage forming. The abdomen in pregnancy without asking a clinician, since ACOG advises against saunas and hot tubs early in pregnancy because raised core body temperature is associated with birth defects. Over the eyes. And never while you are asleep.

Heat comforts, it does not diagnose. Pain with numbness or weakness, pain that is worse at night, or a hot swollen joint with a fever, needs a clinician rather than a warmer pad.

Questions people ask

Where do you put a heating pad for period cramps?
Across the lower abdomen, below the navel, over clothing. If the ache sits in your lower back too, heat there as well. Mayo Clinic lists heat on the lower abdomen as a way to ease cramps, and trials of heated patches and wraps found relief comparable to common painkillers.
Can you put a heating pad directly on your knee?
Wrapped around the knee is better than pressed onto it. Cover the muscle above the kneecap and the soft tissue below, and keep the pad off the kneecap itself, where there is very little tissue between skin and bone. If the knee is swollen or hot, use ice instead.
Where do you put a heating pad for lower back pain?
Across the lumbar curve, on both sides of the spine, from the top of the hips up about a hand width. Use a wrap that holds the heat in one band rather than a flat pad that slides, keep fabric between pad and skin, and follow the heat with gentle movement.
How long is too long to have a heating pad on?
Past about 30 minutes on one spot, or any use while asleep. Placement and timing are the same decision: a well-placed pad on muscle covers more area at a lower temperature, and a badly placed one concentrates heat where it will be noticed tomorrow.
